(The language in this agreement has been approved by the Pennsylvania Attorney General's Office and cannot be modified.)

Step 2. Complete the agreement form in its entirety. Only someone with legally binding authority at your school should complete the form. Do not leave any fields blank.
Step 3. Be accurate. Follow the Important Instructions below.
Step 4. Return your completed agreement form by U.S. Mail. Sign the completed form in ink and send:

Important Instructions

  • Make sure your answers are consistent. For example, DO NOT use "ABC High School" in one field and "ABC State" in a different field.
  • Complete ATTACHMENT A Request for Authoritative Source Status (page 8). We must have this document to grant access.
